Immediate Family Death Benefit:
Provides a cash benefit (based upon length of continuous membership in AZCPOA) in the event of the
death of a Spouse, or a dependent child under the age of 18. The amounts will be paid as follows:
1. $250 for a member with up to 6 months of membership
2. $500 for a member with 6 months to 3 years of membership
3. $750 for a member with greater than 3 years membership
Line of Duty Death Benefit:
The Line of Duty Death Benefit policy will cover any AZCPOA member who dies in the Line of Duty and will
pay $10,000 to the designated beneficiary.
Special Note:
Member must have been a member for at least one pay cycle before the Immediate Family Death Benefit
and the Line of Duty Death Benefit will be paid.
